The advanced preperitoneal repair is the most advanced inguinal hernia repair available. It combines the benefits of laparoscopic hernia surgery, in terms of mesh placement, with the safety and simplicity of open surgery. This leads to faster hernia recovery than either laparoscopic or traditional inguinal hernia surgery.

Patients who have the newer, advanced preperitoneal repair have less pain than patients who undergo laparoscopic inguinal hernia surgery. Using a small 1.25″ incision, a thin mesh is placed on the inside of the abdominal wall in the same location as the laparoscopic repair. Since the mesh is placed on the inside of the abdominal wall, patients benefit by having less pain, and a quicker return to normal activity, in the same way as if the repair was done laparoscopically. Because technically this is an open repair, patients benefit from added safety as well as less anesthesia than laparoscopic surgery. In patients with a body mass index (BMI) of less than 28, the advanced pre peritoneal repair can be performed under sedation with the aid of local anesthesia. By avoiding general anesthesia, the risk of post-operative nausea and vomiting is greatly reduced, and patients are able to leave the hospital and recover from the comfort of their homes much quicker.
The most important advantage of the advanced preperitoneal repair is in the significant decrease in chronic postoperative pain which can plague as many as 10-15% of traditional hernia patients.
